FINRA requires fingerprinting as part of the background check process for broker-dealers, registered representatives and other securities industry professionals.

Midwest Identity Services provides professional FD-258 ink fingerprinting completed to the standards required for FINRA submissions, including proper rolling technique, required field completion and quality review before every card leaves our office.

If your sponsoring firm or compliance department has instructed you to complete FINRA fingerprinting as part of a Form U4 or Form U5 filing, this service is designed to meet those requirements. Appointments are available for individual applicants and can be coordinated for multiple registrants from the same firm.

What to expect

The FINRA fingerprinting process

Most appointments take 15–20 minutes. Accuracy and quality are prioritized at every step to minimize the chance of rejection.

  1. Book your appointment & confirm requirements

    Book online or call (816) 442-0295. If your firm has provided specific instructions, a CRD number or a reference form, have those ready. Calling ahead lets us confirm any firm-specific requirements before you arrive.

  2. Identity verification & intake

    We verify your government-issued photo ID and collect all required applicant information, full legal name, date of birth, Social Security number and any firm-provided reference numbers. We review your firm’s instructions at this stage so all fields are completed correctly from the start.

  3. Ink fingerprint capture on FD-258 cards

    Using professional fingerprinting ink and technique, we roll all ten fingerprints onto the FD-258 card. FINRA submissions require heightened attention to rolling technique, ink coverage and ridge legibility.

  4. Quality review before you leave

    Every completed card is reviewed under proper lighting for ridge clarity, complete impressions and correct field information. If any print or field does not meet quality standards, we redo it on the spot, no extra charge.

  5. Cards in hand, submit per your firm’s instructions

    You leave with completed, quality-reviewed FD-258 cards ready to submit. Whether you deliver them to your sponsoring firm or submit them directly as instructed by your compliance department, the cards are ready to go. We do not submit to FINRA on your behalf.

Why does FINRA require fingerprinting?

FINRA requires fingerprinting as part of its background check process for securities industry professionals. Under FINRA Rule 3110(e), member firms must investigate the background of associated persons, and fingerprinting is required by the Securities Exchange Act of 1934 for certain categories of personnel. The fingerprints are used to check criminal history records through the FBI.

Do you fingerprint bank employees, executives and insurance professionals too?

Yes. Beyond FINRA registrants, we fingerprint bank and credit union staff, executives and board members, insurance producers and other financial professionals. Requirements differ by employer and regulator: some accept FD-258 cards, others require electronic submission through a specific vendor. Send us your instructions and we will confirm the correct method before your appointment.

Do you submit fingerprints to FINRA on my behalf?

No. We provide the completed FD-258 fingerprint cards only. Submission to FINRA is handled by you, your sponsoring firm or your compliance department, following the instructions they have provided. We do not submit on your behalf, control processing timelines or have access to FINRA’s systems.

How many FD-258 cards do I need for FINRA?

Most FINRA submissions require two FD-258 fingerprint cards per individual. However, some firms or situations may require additional copies. Check with your sponsoring firm or compliance department before your appointment and let us know when you book so we prepare accordingly.

What does FINRA fingerprinting cost?

FINRA fingerprinting uses FD-258 cards: $45, plus $20 for each additional card. Cards and a mailing envelope are supplied. Mobile appointments add a travel fee quoted before booking, and we confirm all costs before your appointment begins.

What information do I need to have ready for the appointment?

You will need your full legal name, date of birth, Social Security number, and any firm-provided reference numbers or CRD numbers. Also bring your valid government-issued photo ID and any instructions or forms provided by your sponsoring firm or compliance department.

Can my firm schedule fingerprinting for several registrants at once?

Yes. We accommodate coordinated appointments for multiple registrants from the same firm, either at our office or via mobile service at your firm’s location. Each individual is processed separately with their own completed, reviewed FD-258 cards. Firms with ongoing hiring can also set up a corporate fingerprinting program.

Do you offer mobile fingerprinting at our office or for our executives?

Yes. We offer mobile fingerprinting at firm offices across the Kansas City metro, and elsewhere in Missouri and Kansas by arrangement. It is particularly useful for executives and for firms coordinating several registrants. Group pricing is quoted in writing before we book, based on headcount, card types and location. I am registering in more than one state. Do I need to be fingerprinted for each one?

Not always. Our digital print-to-card system can capture your fingerprints once and produce multiple FD-258 or agency-specific cards when permitted by the receiving agency. Multi-state print-to-card is $60, plus $25 for each additional printed card. See multi-state fingerprinting.

My previous FINRA submission was rejected, can you help?

Yes. Bring the rejection notice when you come in, it will cite the specific reason the submission was rejected. We will prepare new FD-258 cards with direct attention to the cited issue, whether that is poor ridge clarity, incorrect field completion or card handling problems. We review every card before you leave.
