How KSBN fingerprinting works
KSBN uses a mailed FD-258 card. You send the card, the waiver and the board’s fee to KSBN yourself.

Does KSBN accept FD-258 cards rolled at a private fingerprinting office?
Yes. KSBN’s instructions say the prints do not have to be taken by a law enforcement agency. We roll a standard FD-258 card with KSBN’s ORI number and sign the KSBN waiver as your technician.
What is the KSBN waiver, and who signs it?
It is KSBN’s fingerprint waiver agreement. You sign it, and the person who rolls your prints signs it too, dated the same day as the prints. Bring it printed and unsigned, and we complete it at your appointment.
Do you mail my fingerprint card to KSBN?
No. KSBN asks applicants to mail the card, waiver and fee themselves. We give you the completed card and a mailing envelope.
I’m also applying to a nursing board in another state. Can you do both?
Yes, when the other board accepts printed cards. With Multi-State Print-to-Card ($60, plus $25 for each additional printed card) we capture your prints once and print cards for each board. Some boards, including Missouri’s, use their own electronic vendor, so send us their instructions first.
